It's not that tough
A virus running on Windows is, indeed, a problem. So is a misconfigured Linux machine that serves as an open relay. Now throw in unknown vulnerabilities in hardware devices (internet refrigerators, etc.). All might be used to spew spam, without the knowledge of their owners.

ISP's don't need to reconfigure all of their networks to handle this, they just need to block egress port 25 traffic. Many already are. The way mail is currently handled on the internet only needs a slight tweak, and that is to separate initial mail submission from mail transport (using a different port for IMS). Many already do this for their own security reasons.

It's not that tough.
