Irresponsible ISPs are to blame
ISPs need to block outgoing port 25 traffic, except to their own SMTP servers. To allow workstations to connect directly to remore SMTP servers is just plain irresponsible.

If a home user on a dial-up, DSL, or cable modem needs to use a remote SMTP server for e-mail (e.g. for their company), the remote SMTP server needs to be configured to allow initial mail submission on a different port (the most common for this are ports 465 or 587). This is not difficult to do!

If a home user absolutely has to have their own SMTP server running at their house, then they need to get a static IP address from their ISP, and the ISP can then easily track them down in case of abuse.

One of the key problems with SMTP is that, as originaly designed, initial mail submission (client sending a message) and mail transport (SMTP server to SMTP server) both use the same port. These are different activities, and spammers are installing trojans that take advantage of it.
