You're forgetting a couple things
Primarily the registry, the startup group in the start menu, and the config files in sysedit.

One quick look in those places is all it takes me to find adware, viruses, trojan horses, etc. Most are in HKEY_LOCAL_MACHINE/SOFTWARE/Microsoft/Windows/CurrentVersion/Run, though some will show up in HKEY_CURRENT_USER/SOFTWARE/Microsoft/etc...

Once you find them in there, find the executable itself and delete it, then delete the key. Takes all of 2 minutes, unless the executable is running and you can't kill it (yet ANOTHER major problem I have with Windows, allowing programs to run that can't be killed) in which case you have to reboot in safe mode, which might add another 2-5 minutes.
