AIUI, the issue arises when LousyBank Inc. launches a new window or maybe a frame with secret title "LousyBankLoginForm". HackYou.com, in the background, can then fill in its own choice of content in "LousyBankLoginForm".

Windows XP SP2 pops up a "say, what?" message when you navigate from a Web site outside your "trusted" set to one inside; that helps. But who sets up trusted? Well, I did - because I set JavaScript to off for untrusted sites, because of hacking, even with SP2, but I use a few sites that require it. So they're trusted.
