The simple fact is that Windows 3.1 and previous were written in a DOS pre-security era. Nothing was done to improve this with Windows 9x/ME because they were single user systems with no underlying security.

The No-Brainer IS THE EXACT REVERSE. When Microshaft decided to compete with the mainframe/mini market, they hired away a good chunk of the core NT programmers from Digital Equipment. The systems in use at that time for mainframe/mini computers were multi-user/multi-tasking systems that always had security emphasized first and the Windows NT crew used that model to create the "NEW TECHNOLOGY" version of windows. It was written from a pre-MS-DOS security-based era.

The problem came about because it wasn't a very good gaming OS, video wasn't fast enough, users had to understand something about security, device drivers weren't always available. So in the ensuing versions, security was unbolted, subverted, drilled through and upended so things like browsers, et. al. could work on it. As a matter of course, the original design team quit.

It got handed over to the Windows9X crowd for improvements (security breaches) and you have the sorry mess you see today. You cannot simply bolt a patch on the USS Cole to cover the Al Quaida bomb-blast hole. You cannot simply cobble together security overnight to fix the rat infestation that Windows NT became.
