What about trasparent proxing ?
My opinion is that actually 90% of viruses spread by email. So, what about protecting mail servers using trasparent proxies at ISP sites.
It will be easy to redirect connections to ports like the 25 to a virus protected relay.
ISP customer has no need to connect to other smtp servers directly.
Furthermore it will be easy for the ISP, knowing the real email address of his customer, by knowing the user associated with the IP, to inform him automatically about infections.

My 0.02 cents....
