The Real Problems
The real problems are:

1. The hackers and virus writers. There's no one forcing them to write code that invades other people's PCs or ties up their time in clearing out their mail boxes of the attempts to send them a worm. In my opinion they are criminals, each and every one.

2. The users who even after the amount of publicity that worms and viruses have had still go ahead and open these attachments. WHEN will they learn? I know that I got pissed at someone in my household one night and raised my voice (stopping short of really yelling) to emphasize my point "No, this is NOT a message from your brother. It's a message from a worm on his computer. There is no such thing as a person you can trust when you get a message with no personalization in it and it comes with an attachment.". She finally got the point.

OS vulnerabilities are third and last. Anything as complex as a major OS will have flaws. There's no excuse for not fixing them but I hold the people who write "exploits" as the most culpable.
