I can't answer the first part, there are vested interest that cloud the picture way too much. My take is that local governments (councils, etc.) should own conduits that underground services run through. These should be installed when stormwater, sewerage, water and other services are installed. They can then allow any licenced technician to install cabling, much like happens now with electrical cable in your house. Any company installing cables pays a rent to the council. Then anyone can provide infrastructure and compete with the Bell's. I would also require that anyone building local infrastructure can't provide services unless there is no competitor in the local market. This restriction would be dropped once sufficient competition existed.
Ownership of the conduits could also be sold off when conditions are right.
As to the second part, the cost of installing local lines is prohibitive - the only way you can justify putting them in is if you are guaranteed to get rental from the line for quite some time. If three service providers run a cable to my home, only one will get paid for it. That just doesn't make sense. If the above system is implemented, the cost of installing local lines becomes way cheaper, and service providers can compete to provide service over a number of infrastructure providers.
They can't raise costs to other carriers above the cost of installing a local loop, and if there's a public conduit available, that can be done quite cheaply (compared to now).
