Even certification isn't always enough
The problem with releasing Microsoft's patches ASAP is that you can't guarantee you won't be trading a headache for an upset stomach, as they used to say in the old commercials.

We have a very short certification period for MS patches here, but even so we occasionally get stung by problems created by the patch. For example: after blowing out MS04-011 to our entire enterprise, we wound up with nearly 1000 NT systems with disabled mice and keyboards and/or BSODs because of an incompatibility with Microsoft's own drivers (see http://support.microsoft.com/default.aspx?scid=kb;en-us;835732 for the gory details).

So system admins are often caught between Scylla and Charybdis, so to speak. If they don't push a patch out soon enough a worm gets in. Push it out too soon and some systems may be disabled. The real issue is Microsoft's inability to come up with either secure software or patches that have been properly regression tested.
