Commercial anti-spam is a waste of money.
We use http://spamassassin.apache.org/ on our incoming email gateway. It rejects 83% of email as spam outright, labels as spam but delivers 9%, and delivers as unmarked 8%. Volume: ~350,000 messages/week. Not much spam slips through, and false positives are rare.

While it was admittedly a bit on the hard side to setup originally, ongoing maintenence is very, very low - it just works.
