Underestimated user stupidity
I think you underestimate the stupidity of the average user. I don't have time to go into details, but I've had enough issues with people that don't even know the very basics of internet security opening things they should never touch. Granted, 99% of the time, our anti-virus catches it before it does damage, but just the nature of it shows basic lack of skill.

As for "why don't you train them!?", our company won't support the time spent on such a "trivial" thing; to them, we're IT, we can make viruses and keyloggers by doing magic, and spending their money and our (unpaid) overtime, of course. We have our security settings set so hard that people can barely do their jobs, mainly to protect themselves... and us.
