How about access lists?
If every ISP controlled port 25 to and from thier internal mail servers so that the only SMTP trafic allowed is either going to or from thier servers you wouldnt see any more bogus headers in the e-mail and it would be very easy to track.

After that it would be pretty academic as there are plenty of programs out ther to track useage based on account useage the spammer would be very easy to spot and boot.
