Forget the OS
Don't get fixated on the OS here. So for the OS zealots here this could have been any OS so keep your pants on.

The problem could easily have been an application level issue where the attacker could have found a hole via SQL-Inject, XSS or any other number of app layer issues.

Doesn't matter what OS, how many firewalls or how locked down the infrastructure was. You leave one hole open from some poorly written app and the gig is up.
