And I would agree with you. However, imagine an OS that
can even prevent using the extra motions!

Mount the users write-able directories (eg /home, /tmp,
others) directories as a separate partition with the NOEXEC
option.

Then even if the users saves the attachment, sets the
executable bit, they still won't be able to execute anything.
Protects from user software sourced by ftp, http, disk, etc.
