I hope your mother never makes a mistake...
Most rules and regulations, standard operating procedures and safety nets have been established as a result of a threat or incident that brought to light the failing. You solve the problem so it doesn't happen again, and prevent the other ones like it that might. This one should have been prevented, no argument there, but I bet that not only D&T but a dozen other such companies will not let it happen again.
