Make the companies financially and personally responsible
If you want to stop most security breaches, make the companies who permitted the breach financially and personally responsible:

- Financially. You did it, you pay for the clean up. That way companies that use their services, who weren't responsible for the break, don't have to pay for it. That way individual users who indirectly use their services, who weren't responsible for the breach, don't have to spend their money and time to fix it.

- Personally. Like Sarbanes-Oxley, make the executives of the companies responsible for the breach personally responsible. Read that as they must personally pay fine and/or do jail time if their company was the one that let the cat out of the bag.

Think about that last one for a moment. As it now stands, these companies that are responsible for the breach can hide in background anonimity and carry on with "business as usual". There is no motivation for them to change their business practices or their security measures. Turn that around making them face stiff fines and possible jail time. Now there is strong motivation for them to do something about a breach. Do that and the landscape will change dramatically and quickly.
