2. What systems were involved?
3. When was the compromise actually discovered?
4. How long had the compromise been in effect?
5. What steps are being taken to correct the security infraction?
6. Who is being held accountable for this?
7. What systems were involved?

These are questions that need to be answered and I am sure there are more.
