Software that was not broken would presumably protect you from yourself, and would not need to be protected from itself (as it was not broken).

Read what I said more carefully. Anti-malware does not protect you from software, it protects you from yourself. Airbags and seatbelts don't protect you from faulty cars, they protect you from faulty drivers. See the difference?

Very arguably. If a user runs the Kama Sutra attachment, Windows merely does what the user is asking it to do: run a program. All OSs do that in their default configurations, they simply run programs that users ask them to run. Anti-malware, if anything, breaks the OS by stopping an OS from doing what it was designed to do: run software whenever the users asks it to. No default install of any home OS currently has any defense against any of the damage caused by Kama Sutra. They all allow users to run programs and they all allow users to delete their own files.

Personally I do not think that such a list could ever be completed, or even generalized to fudge it being completed.

This goes without saying. The best defense is common sense just like it is safer to drive defensively than it is to rely on airbags and seatbelts.
