Simplest solution

Mark up document as usual. Print it. Scan it in a copier that turns it into a PDF, then distribute the PDF. Since all the scanned information is graphical in nature, black boxes over graphics and blocking out text would work, and there's no bothersome text embedded into the PDF file. And it allows people to work pretty much as they always have.

Makes me seriously wonder who's making IT recommendations at NSA.
