Save and execute no fix
Forcing the users to save an attachment before executing it isn't really going to fix anything.

The reason I say that is because once users get used to the save/execute routine, THAT'S EXACTLY WHAT THEY WILL DO.

After all, MyDoom has to be run by the user before it executes. Although the bit about unicode not being displayable is clever, I must admit, at least for the average user.

It all comes down to the user opening an attachment they weren't expecting. NOTHING technical is going to stop that.
