Lack of communication.
The software we sell advises users to backup frequently. It allows them to store the data on a local workstation or on the server.

About 25% of the backup problems come when there is a data/system crash and there is NO backup. IT was informed to put the data on the nightly backup schedule, but never got around to it ... for several years!

It is up to every user to verify that their data IS being backed up and to specify how long those backups are being archived. We've actually found sites where today's backup is recorded over yesterday's backup! If a data problem occurs today and is then backed up, you have no "good" backup to go back to!

I'm amazed at how casual users are about backup and how little they do to manage it. We are also very dissapointed in the quality of the restore programs. Some can only restore the entire backup, not pieces of it to recover the one folder of data that is corrupted.

You are right, bigger and better methods are needed for both backup and restore and users need to get more involved in the process.
