Surely this is a joke
"One of the suggestions on Gates' antispam checklist is
setting those sending e-mails a simple brainteaser, or
asking their PCs to do an easy computation"

What about when I send out email to authorized mailing
lists such as customer's monthly invoices and technical
notices? Even for a small number of customers in my
business this would be a pain, but for the 10,000s for my
ISP it would be unbearable.

"If you receive an e-mail from an old school friend, and
you're happy to receive it, the sender doesn't pay. If it's
another offer of a porn subscription, you reject it, and the
spammer is forced to cough up."

So I'd need to open an authorized account before I can
send mail somehow connected to a bank account? Perfect
for those countries looking to impose a stamp like charge
for emails.

However, I feel sorry for all those windows users hacked
into to forward spam, your windows habit could become
quite expense (or does he propose an international court to
resolve such problems?). Imagine opening those email virii
which mail to everyone in your address book. Fortunately
MS provides protection in their EULA for such charges, no
wait seems not.

"He acknowledged that [mail] filters have their part to play
in the spam struggle but said he believed that they
wouldn't ultimately solve the problem."

The mail filters are the only solution to this problem until
uniform anti-spam laws are put in place around the world
and are widely enforced. Tools such as SpamAssassin have
become extremely effect against spam.

Bill Gates should be knighted for his services to comedy.
