The vast windows-based zombie bot armies that are out there. I'd guess therefore that patching doesn't happen as often as one might hope.

Also, as has already been raised, larger institutions have to test patches to see if they break stuff. They have a longer exposure risk.

It comes down to doing the right thing. Checking for patches, having enough information about any downside of patching if they are available, and being lucky enough not to get zapped by a "zero day event", and getting the patches in place in a timely manner.
