Damn the security, full speed ahead!!
Let me guess...the programmers knew they should be checking the code, but no one would commit the extra resources to code in the checks and test them, because it wasn't absolutely ciritcal to getting those ATM fee dollars roilling in.

I don't feel even slightly sorry for a bank which loses money that way. It's their customers, who have no way of knowing the bank has been sloppy, who will take the brunt of it. This smells like a class action suit just waiting to happen.
