Users tend not to have a clue about the technologies that they use and make a lot of assumptions. Its not their fault, they are accountants or sales people etc., not security professionals.
In every one of the listed examples, the users were using features in ways that they weren?t intended to be used. Companies must invest in the tools that work and educate their users on when to use them and why. In addition users need to be held accountable for their actions.
There are many great products on the market that prevent forwarding, printing, copying etc. Giving users access to the tools and a bit of education would go along way. Adding additional tools that automatically protect content based on rules (content, recipient, sender, etc) would go a long way to stopping accidental disclosures
